  • Coordinated care between nursing and rehab staff to ensure resident needs and to ensure residents needs and to ensure that  individualized goals and plans of care were developed
  • Scheduled,  completed and transmitted all MDS assessments to state data base in a timely manner to ensure appropriate billing

  • Conducted Medicare meetings to ensure proper documentation to meet Medicare guidelines

  • Direct patient with long term patients and acute care setting.
  • I mainly worked on acute rehab side where I started IVs, administered medications, wrote doctor orders, assisted doctors with patients rounds, checked PT and INR values, performed treatment to wounds (especially wound vacs and pressure ulcers), assisted patients with ADLs, performed peritoneal dialysis, g-tubes, monitored lab values.
  • Worked directly with patients and families to assist patients to get their peek potential.
